New Families

1. Create a FinalSite account - https://lacschool.fsenrollment.com/users/sign_in
2. In FinalSite, complete the "Application Form". 
3. After completing the application, you will receive and email stating that you have been accepted and can begin the enrollment paperwork. The following paperwork is needed for new students:
   • Tuition Contract (handled through St. Simon Business Office - Amy McGraw)
   • Birth Certificate
   • Enrollment Acknowledgements and Agreements
   • Medical Emergency Contact / Health Information
   • Concussion Form
   • Immunization Records
   • Family in Partnership Agreement Form
   • School Handbook Signature
   • Health Appraisal (for preschoolers, young 5's and kindergarteners)
   • Oral Health Assessment Form (for kindergartners)
   • Vision and Hearing Assessment Form (for kindergartners)
   • Parish Support Form (for active Catholic parishioners)
   • Sacrament Records (optional)
   • OTC Medication Form (optional)

Returning Families

1. When enrollment is open for the next school year, you will receive an email prompting you to begin the enrollment process.
2. Log into FinalSite and begin the enrollment paperwork. The following paperwork is needed for new students:
